Philadelphia, Pa., November 16, 2021 — Jefferson Health and BAYADA Home Health Care, a leading nonprofit home health care provider, today announced that they have entered into a definitive agreement for a joint venture that will catalyze post-acute care and digital health transformation for Jefferson patients in New Jersey and Pennsylvania.
Preliminarily referred to as ‘Jefferson Health at Home’, the joint venture intends to leverage the talented employees and innovative spirit of both organizations to reshape, reimagine and revolutionize the future of how healthcare services are delivered at home with specific emphasis on seamless transitions in care and easy access to in-home and virtual care to create the best patient experience in the industry.

Now, with BAYADA and Jefferson aligned, we’re able to leverage their best post-acute-care practices across an expanded geographic footprint while lowering the total cost of care without compromising access or quality.”
Jefferson Health at Home’ will combine both organizations’ management, expertise, operating platforms, clinical excellence, and reach to enhance the collective impact on the continuum of care offered to Jefferson patients across Bucks, Montgomery and Philadelphia counties in Pennsylvania and in Camden, Burlington and Gloucester counties in New Jersey.

The partnership will have joint leadership and governance from both Jefferson and BAYADA. Jefferson’s contributions to the joint venture include its home health and hospice services at Jefferson Health New Jersey and Abington Jefferson Health, while BAYADA will draw on its scale and management systems to oversee the joint venture operations.

The agreement, which is expected to be finalized in Spring 2022, also includes an academic affiliation which will create new training opportunities for Jefferson students, particularly those at the Jefferson College of Nursing.
