Before Felicia, Debra Chapman’s home was not a safe place to be alone. Debra, who lives near the Jersey shore with her husband of 54 years, Dave, had reached a point where getting up and down and around the house carried real risk.

Over roughly six months, Debra fell 26 times. Dave, unable to lift her on his own, improvised a system with an air mattress just to get her off the floor, and the couple grew used to the sight of police and EMTs in their living room.

“That was getting old,” Dave said. “And dangerous, obviously.”

Debra’s path to a safer home life began with a hospital stay and several weeks in a nursing home—a difficult experience. When she was ready to come home, BAYADA Home Health Care sent a registered nurse clinical manager to assess her needs, recommend home modifications such as a lower bed, grab bars and ramps, and build a care plan. The next step was finding the right caregiver to carry it out.

That is where BAYADA Home Health Aide Felicia Washington came in. Her client services manager knew she could provide the care that Debra needed: Felicia is patient, skilled with transfers, and strikes the right balance between encouraging clients’ independence and stepping in when they need help. Within the first month of Felicia caring for Debra at home, both women knew they had found the right match.

The Right Match, The Right Hours

What makes this story special is not just the right caregiver, but the right timing. Debra’s personal care services are powered by an AI-enabled, nurse-led care model called Enhanced Quality of Care. Its technology helps to track patterns that identify higher-risk periods over the course of an individual’s day. BAYADA nurse managers interpret this data to make targeted, early interventions—such as scheduling caregivers to be present during the hours that matter most.

For Debra, her high-risk periods are transfers—getting in and out of bed and general mobility challenges. Felicia takes an active role in fall prevention to reduce Debra’s risk. When Debra pushes herself to do more than her body can safely manage, Felicia steps in to help her recognize her own limits and take precautions before a fall happens.

A Turnaround in Quality of Life

“I have BAYADA’s very best person here, and I hope she never leaves. She’s just a special person to me, and I love her dearly. Made a big difference in my life.”

— Debra Chapman, BAYADA personal care client

Debra’s turnaround speaks for itself, and she attributes it to Felicia’s prevention measures. With Felicia present, Debra’s monthly fall rate has plummeted by about 96 percent, going from an average of one fall per week to only two falls in the past year. And those two falls incurred no injury because of her home safety modifications.

Dave feels a shift in his independence, too. “I couldn’t be any more confident in Debbie’s safety when I’m away,” he said.

Debra and Dave are back to enjoying the things they love. With Felicia’s help, Debra has returned to lunches with friends, family outings, church choir and Bible study, and even a family road trip to Florida—the kind of ordinary life that once felt out of reach.

A Care Model Built for Outcomes Like This

Debra’s story is one family’s version of what BAYADA is building to scale: a nurse-led, AI-enabled care model that identifies risk early, matches clients with the caregiver suited to their specific needs, and schedules care when it is needed most. BAYADA is changing the future of home care one data point, one intervention, and one improved outcome at a time—and for Debra and Dave, that change is measured in a life that once again looks like their own.
